To file a warranty claim, you will need the following information:
Photo or File 1: Proof of Purchase
Please provide a photo or scanned copy of your purchase receipt.
Your receipt must include:
- Retailer name
- Original purchaser's name
- Full date of purchase (MM/DD/YYYY)
- Purchased item(s)
- Purchase price for each item
If you do not have a receipt, we may be unable to proceed with your claim.
Photo 2: Full View of the Power Base
Please remove the mattress and provide a clear photo of the entire power base. This photo is best taken from the foot of the bed and should show the complete base from head to foot and side to side.
Photo 3: Area of Concern
Please provide a clear photo or video showing the issue you are experiencing.
- For broken, damaged, or missing components, please provide clear photos of the affected area.
- To demonstrate noise concerns, please provide a video file showing the issue.
- To demonstrate stripped threads on a power base leg, please provide a video file clearly showing the affected leg and the threading concern.
Providing a video for these issues helps our team accurately evaluate the condition of the power base and determine the appropriate next steps.
Photo 4: Serial Number
Please provide a clear photo of the power base serial number.
Depending on your model, the serial number may be located:
- Inside the remote battery compartment on a white sticker
- Along the metal frame of the power base
- On the QR code tag attached to the base
- In the owner's manual
Note: Newer power base models may have the serial number on a separate sticker or tag rather than on the law tag. A photo of the serial number is required to review your claim.
