To file a warranty claim, you will need the following information:
Photo or File 1: Proof of Purchase
Please provide a photo or scanned copy of your purchase receipt.
Your receipt must include:
- Retailer name
- Original purchaser's name
- Full date of purchase (MM/DD/YYYY)
- Product purchased
- Purchase price
If you do not have proof of purchase, we may be unable to proceed with your claim.
Photo 2: Foundation and Support System
Please remove the mattress and provide a clear photo of the entire foundation from head to foot and side to side.
In addition, please provide a photo of the bed frame supporting the foundation with the foundation removed. The photo must clearly show:
- The entire bed frame
- All support legs
- Center support legs, if applicable
This information allows us to verify that the foundation is being used on an appropriate support system and helps rule out support-related issues that may contribute to product damage or performance concerns.
Photo 3: Area of Concern
Please provide a clear photo of the area you believe is defective.
If the issue involves squeaking or noise, please upload a video instead of a photo. For this video:
- Remove the foundation from the bed frame.
- Place the foundation directly on the floor.
- Demonstrate the noise or issue being experienced.
This helps us rule out the bed frame or support system as a contributing factor to the issue.
Photo 4: Foundation Law Tag
Please provide a clear photo of the foundation law tag.
The law tag is typically located on the side of the foundation near the head or foot of the product.
